Subject Access Request (SAR) & Data Subject Rights Form Version 2.1

Welcome to Solars online SAR and Data Subject Rights Form

This form provides the ability for the Data Subject to raise either a Subject Access Request (SAR), and/or execute one or more of their Data Subject Rights. Please complete this form if you wish to see your data, or exercise one or more of your rights. As part of these requests, and Information Security governance, you will be required to provide proof of your identity.

For SARs, your request will be processed within 30 calendar days upon receipt of a fully completed form and proof of identity. For action of rights, Solar will aim to process within 30 calendar days (Both SARs and action of rights being extendable in cases where requests are complex or numerous).

Section 4: Personal Details if the person acting on the Data Subjects behalf

Please complete this section of the form with your details if you are acting on behalf of someone else (i.e. the data subject). If you are NOT the data subject, but an agent appointed on their behalf, you will need to provide evidence of your identity, as well as that of the data subject and proof of your right to act on their behalf.

Section 5: Proof of identity

In order to be able to process your request, we require proof of your identity before we can respond. Proof of your identity should include a COPY of two documents as a minimum, such as your birth certificate, passport, driving licence, official letter addressed to you at your address e.g. bank statement, recent utilities bill or council tax bill. The documents should include your name, date of birth and current address. If you have changed your name, please supply relevant documents evidencing the change.

Administration fee:

Under the GDPR, a request for personal information through this process is free unless the request is ‘manifestly unfounded or excessive’ e.g. Solar can charge a ‘reasonable fee’ for multiple requests. This is also the case for Data Subject Rights.
